Hipertensi merupakan penyakit kronis yang memerlukan pengelolaan jangka panjang untuk mencegah komplikasi dan meningkatkan kualitas hidup pasien. Manajemen diri yang kurang baik berkontribusi terhadap tekanan darah yang tidak terkontrol dan penurunan kualitas hidup. Pengabdian masyarakat ini bertujuan untuk meningkatkan manajemen diri dan kualitas hidup pasien hipertensi melalui edukasi kesehatan di wilayah kerja Puskesmas Cepiring. Metode yang digunakan adalah intervensi edukasi dengan desain pre-test dan post-test. Hasil menunjukkan adanya peningkatan signifikan pada kemampuan manajemen diri dan kualitas hidup pasien setelah intervensi. Edukasi kesehatan efektif dalam meningkatkan kemampuan pasien dalam mengelola penyakit dan kesejahteraan hidup. Program berkelanjutan sangat direkomendasikan.
